ACCORD試験の脂質管理部分発表、糖尿病治療にフェノフィブレートのスタチン併用効果を認めず
今週のNew England Journal of Medicineに、ACCORD試験の脂質管理部分(ACCORD Lipid)の結果が報告されました(Effects of Combination Lipid Therapy in Type 2 Diabetes Mellitus: The ACCORD Study Group)。
Conclusions: The combination of fenofibrate and simvastatin did not reduce the rate of fatal cardiovascular events, nonfatal myocardial infarction, or nonfatal stroke, as compared with simvastatin alone. These results do not support the routine use of combination therapy with fenofibrate and simvastatin to reduce cardiovascular risk in the majority of high-risk patients with type 2 diabetes.
主要な成績は下記になります。これは(糖尿病)全体で見た場合で、確かにfenofibrateの併用効果は見られていません：
一方、高脂血症者に絞って見た場合には、有意差こそなかったものの、効果がありそうな傾向を示していた、とのことです。下記の表は、これまでの他の臨床研究の結果とともに一覧表にしたものです：

フィブラート系薬剤は元々、高脂血症の治療薬ですし、「『全糖尿病』を対象にしても効果が見られなかったが、高脂血症例に限定すれば効果はあった」というのはしっくり来る内容です。
